Registration Information


Before completing the registration form, attendees are requested to read the following information. Please print clearly or type all information when filling out the attached registration form. Fill in all requested information completely. Failure to provide full and accurate information will delay your registration. 

1) Please Print My Badge to Read: 

This information is for printing your badge only. Please list your preferred first name this may be a nickname or your complete name as it appears in the member records. Please provide your organization's full name and finally, the city, state/province, and country where you work. 

2) Registration mailing address: 

Please give us a preferred mailing address so we may confirm your conference registration. It is important to also give us a phone number, fax number, and an e-mail address if you have one. 

Note: If you have changed your address, here is an opportunity to inform SLA. Please check the appropriate box if you wish us to update our records. 

If this is your first SLA annual conference, please check the appropriate box. There is a special "introductory reception" for first-time attendees on Sunday, June 7 at 1:30 p.m. Check the program for more details. 

If you have agreed to be a speaker, moderator, panelist, or facilitator in one of the many conference activities, please check the appropriate box. 

3) Ticketed Event Orders: 

List the ticketed event numbers in the columns on the registration form found on the back cover of this Preliminary Conference Program

Continuing Education Courses Enter your first choice for a Continuing Education (CE) course on the first line. A line has been provided for you to indicate an alternate CE course should your first choice be over-enrolled. U.S. citizens must provide their Social Security number for CEU record keeping purposes. Please be sure to include the appropriate course registration fee in the last line, Total Amount Enclosed.  

Special note: Students wishing to register for a SLA Professional Development CE course will be charged $95 per course. This rate does not apply to Division CE Courses, Executive Management, or Middle Management courses. 

Conference Ticketed Events Insert the event number and number of tickets for each event with the subtotals in the far right column. Use provided space to order your tickets for the SLA Awards Banquet, President's Reception Fund Raising Event, meal functions, and tours. SLA will not maintain a waiting list for those events sold out. Please check at registration, onsite, to see if additional tickets may be available. Remember check the ticket exchange board at conference. 

4) Registration Fee: 

Fill in the appropriate registration fee amount in the far right column. Fees are payable in U.S. Dollars only. Registratrants will receive written confirmation. No receipts are issued. 

One day registrants are asked to circle the day in the space provided.

* SLA member rates apply to members of Special Libraries Association (SLA), American Association of Law Libraries (AALL), American Society for Information Science (ASIS), Art Libraries Society of North America (ARLIS/NA), Association of Independent Informational Professionals (AIIP), Australian Library and Information Association (ALIA), the Canadian Library Association (CLA), and the Medical Library Association (MLA). 

SAVE MONEY! Join SLA now and pay the member registration fee instead of the non-member registration fee. Complete the membership application form on page XX and mail together with your completed conference registration form to guarantee your savings. You must select the January 1998 membership cycle on the membership form to receive member conference rates and to begin receiving SLA member benefits today. 

5) Total Amount Enclosed: 

Please enter the total calculated amount from all of the above entries. Payment should reflect conference fees only. Payment should be made in U.S. dollars only. Special note: Payment must be received in order for the registration to be processed. Payment for any other SLA products & services should be sent to SLA under separate cover. Envelopes must be postmarked by May 1, 1998, to qualify for the "Early Bird" registration fee. All registrations postmarked after May 1, 1998 are considered and handled as regular registrations. Credit card registrations may be faxed to association headquarters at 
1-202-234-2442. To avoid double billing, fax registrations should not be followed up by mail. Registration forms received at SLA after May 15, 1998 will not be processed before-hand please register onsite. 

Refunds/Cancellations 

Full refunds for conference registration fees, Continuing Education courses, and ticketed events will be made only if cancellation is received in writing and postmarked on or before May 1, 1998. Refunds will be issued on a monthly basis. Credit balances may not be used to purchase additional items on site.

Americans with Disabilities Act 

Please write the director, SLA conferences and meetings, for special requests regarding physically challenging barriers in attending the annual conference. Reasonable advance notification is required to make necessary arrangements. A member of the SLA staff will call you to discuss specifics to facilitate your attendance and participation. Individuals requiring special meals due to medical, religious, or dietary restrictions are requested to write the director, SLA conferences and meetings, indicating which events the attendee plans to attend and detailing restrictions.
