There are three ways to obtain and complete the conference registration form:

• you may complete the online (HTML) version of the form

  The HTML version allows you to fill in the registration form online. Once the form is completed, you must print the pages and mail or fax the form to the address or number below. You may not submit the registration form online.

• you may download the Adobe Portable Document Format (PDF) version of the form

  The Adobe PDF version retains the original format of the registration form. You must download the file and open it in Adobe Acrobat Reader. Print the file from Acrobat reader, then complete the form. Mail or fax the completed form to the address or number below.

  Note: To View or Print a PDF file, you need the Acrobat reader. The software is available FREE, from Adobe. For more information, visit Adobe's web site at http://www.adobe.com.

• you may have the form faxed to you

  The Registration Form can be obtained through our fax-on-demand system by calling 1-888-411-2856 and following the prompts. The form number for the registration form is 502.
