SLA Association Awards Nomination Forms

  • SLA Awards & Honors Nomination Form (used for all association level awards) 

    • Nominations now being accepted for:
      • 2009 Presidential Citations- Accepted Year Round

  • Upcoming Deadlines:

    • 2010 Fellow of SLA- Nominations open September 1, 2009 and close on December 15, 2009
       
    • 2010 SLA Rising Stars- Nominations open September 1, 2009 and close on December 15, 2009

    • All other association-level awards nominations will open on November 1, 2009 and close on January 15, 2010.

 

SLA Committee and Council Recognition Nomination Forms

Tips for Nominating SLA Members for Awards and Recognition:

  • Write applications with each specific award's eligibility in mind. Each nominee will be rated on the criteria for the award category.

  • If you wish to nominate in more than one category, attach a separate application for each award, addressing the criteria for that award, including activities, accomplishments, and contributions which demonstrate the specific criteria for that award.

  • Explain, where applicable, the specific results and impact of the nominee's efforts that strengthen the case for that award. Include details such as dates and locations and performance metrics if available. Avoid generalities such as "made documents easy to find," explain with examples, details, even testimonials.

  • Attach the nominee's bio or resume. Attach relevant supporting documents, including: articles, other commendations, or previous awards and recognition they have received.

  • Please include letters of support which demonstrate the nominee's qualifications, accomplishments, or impact on the association or profession

  • Photos of the nominated person are welcome, and helpful to the SLA Awards Committee in their deliberations.

  • Joint nominations are welcome. Try to coordinate to avoid duplication of content.

Things to Avoid:

  • Submitting one nomination, and checking the box for every award.

  • Sending a list of accomplishments or a resume with no supporting materials or cover letter detailing exactly why that nominee should receive a particular award.
