SLA Awards Nomination Forms

Student and Academic Affairs Advisory Council Merit Awards Nomination Forms

When nominating someone to receive an SLA Award:

  • Write applications with each specific award's eligibility in mind. Each nominee will be rated on the criteria for the award category.
  • If you wish to nominate in more than one category, attach a separate application for each, addressing the criteria for that award, including activities, accomplishments, and contributions which demonstrate the specific criteria for that award.
  • Explain, where applicable, the specific results and impact of the nominee's efforts that strengthen the case for that award. Include details such as dates and locations and performance metrics if available. Avoid generalities such as "made documents easy to find," explain with examples, details, even testimonials.
  • Attach the nominee's bio or resume. Attach relevant supporting documents, including: letters of recommendation, articles or other commendations, or recognition they have received.
  • Joint nominations are welcome. Try to coordinate to avoid duplication of content.
