1. A complete application will consist of a completed application form with a 500-1000 word essay, official transcripts or letter of acceptance, three letters of reference, statement of financial need, in addition to any additional information listed under the eligibility requirements for the scholarship for which you are applying*. Incomplete applications will not be considered.
2. To be considered by the scholarship committee, all items must be postmarked by 30 September.
3. All items must be typed or printed in black ink.
4. Answer all items, inserting "No" or "None" where necessary.
5. Under employment, list all full-time and any part-time or summer work that is pertinent to your library career.
6. In describing your financial needs, you can assist the committee by providing a clear financial picture of yourself. It is not necessary to limit your answers to the questions asked. We realize that financial information is personal, but providing a full explanation will assist the committee members in assessing your financial needs compared to other applicants.
7. Your three letters of reference should address your professional achievements and/or your general character.
8. If applying for multiple scholarships, please select the proper radio button for each scholarship.
9. A complete application must be postmarked no later than 30 September 2008.
